Getting Started

We can never be sure of what tomorrow will bring. A health crisis may affect you or a family member at any time, and advance care planning allows you to have a voice in the care you receive if you are ever unable to speak for yourself.

Having the conversation about advance care planning means reflecting on your values, beliefs, and wishes to make your own health care decisions across your lifespan. By documenting your wishes and communicating them to your loved ones, health care team, and family physician, they will be better able to act on your behalf.

Hospices across Vancouver Island have created the Advance Care Planning Teams to assist individuals, groups, and families in their communities in various ways, including:

  • offering presentations about advance care planning;
  • assisting with workshops on developing advance care plans
  • providing online resources to guide you;
  • educating organizations and professionals in the community about advance care planning; and
  • encouraging individuals to read the BC Ministry of Health’s Advance Care Planning Guide: My Voice - Expressing My Wishes for Future Health Care Treatment

You are encouraged to discuss advance care planning and your wishes with your primary care physician. Let your voice be heard.
